• EAA Membership Form: Use this form to join the EAA and pay your dues.
• EAA Reimbursement Form: Used by coaches and Area Chairs for reimbursement of pre-approved purchases of athletic equipment and supplies. Completed forms, along with receipts, should be mailed to Sean Cover, EAA Treasurer, at 256 Madison Road, Scarsdale, NY 10583.
